How to Complete & File

  • The form must be typed or clearly printed in ink
  • Sign the form
  • File it with the City Clerk

What to Include

Attach any supporting documents you have, such as:

  • Photos
  • Invoices/receipts
  • Proof of ownership
  • Any other evidence
  • If you need more space, attach an additional sheet.

After You File

  • Claims are usually investigated by adjusters for the City’s insurance carrier.
  • You may be contacted by Washington Cities Insurance Authority (WCIA) or Sedgwick (depending on the claim type).

Important Notes

  • Submitting a claim does not guarantee payment.
  • An adjuster will investigate to determine whether the City has liability under applicable laws.
  • If the City is responsible, any payment amount is based on:
    • The level of City liability
    • The level of your liability (if any)
    • The depreciated value of damaged property (not replacement value)
