
If you were injured or your property was damaged and you believe it was caused by the City of Newcastle’s negligence, you must submit a Claim for Damages so the City can determine its responsibility.

How to Complete & File
- The form must be typed or clearly printed in ink
- Sign the form
- File it with the City Clerk
What to Include
Attach any supporting documents you have, such as:
- Photos
- Invoices/receipts
- Proof of ownership
- Any other evidence
- If you need more space, attach an additional sheet.
After You File
- Claims are usually investigated by adjusters for the City’s insurance carrier.
- You may be contacted by Washington Cities Insurance Authority (WCIA) or Sedgwick (depending on the claim type).
Important Notes
- Submitting a claim does not guarantee payment.
- An adjuster will investigate to determine whether the City has liability under applicable laws.
- If the City is responsible, any payment amount is based on:
- The level of City liability
- The level of your liability (if any)
- The depreciated value of damaged property (not replacement value)
